Referral Terms of Reward

1. Eligibility for Rewards
You must submit accurate contact information for the person you’re referring. They must be eligible for ACA coverage and complete an enrollment through a licensed agent at Nest Insurance.2. Consent Requirement
By submitting this form, you confirm that the person being referred has provided permission to be contacted by a licensed agent for the purpose of discussing health insurance options.3. Reward Process
Referrals are reviewed weekly. If your referral enrolls in an ACA health plan, you will receive a gift card via email (usually $25 per enrollment). Rewards are sent every Friday for all confirmed enrollments from the previous week.4. Referral Tracking
You will receive updates on the status of your referrals via email. Submission alone does not guarantee a reward — enrollment must be confirmed.5. Limitations
There is no cap on the number of referrals you can submit. However, rewards will only be issued for referrals that result in confirmed enrollments.
